Neurofeedback vs ADHD Medication

We run a neurofeedback clinic, so you would expect us to argue against medication. We are not going to. Here is the comparison as fairly as we can make it.

You are probably reading this because someone has suggested medication for your child and you want to understand the alternatives before you agree.

We run a neurofeedback clinic, so you would expect us to argue against medication. We are not going to. The comparison below is as fair as we can make it, because a decision this important should not be made on the basis of a sales page.

The case for medication, stated properly

Stimulant medication is the most effective single treatment for ADHD symptoms that currently exists. That is not a grudging admission, it is what the research consistently shows.

  • It works quickly, often within days
  • Effect sizes in head-to-head trials are large, and larger than neurofeedback
  • It is inexpensive compared with almost any alternative
  • It is extensively studied, with decades of data

If your child is struggling badly at school right now, medication is the intervention most likely to help fastest. Any clinic that dismisses that is not being straight with you.

The case against, also stated properly

  • Documented side effects: reduced appetite, difficulty sleeping, mood changes, and in some children tics
  • Research indicates up to 30% of children either do not respond adequately or experience significant adverse effects
  • Benefits generally stop when the medication stops, so it manages symptoms rather than changing the underlying pattern
  • Studies following children over longer periods have found meaningful numbers discontinue treatment

The trial most worth knowing about compared the two directly in children newly diagnosed with ADHD. Methylphenidate produced a large effect on symptoms. Neurofeedback produced a moderate one. The researchers concluded neurofeedback is a promising alternative for children who do not respond to medication or who experience significant side effects from it.

That conclusion is the honest summary of this whole comparison. Neurofeedback is not the stronger intervention. It is the gentler one, with a different profile, and for a substantial minority of children it is the one that fits.

Which situation are you in?

Medication is working and your child tolerates it well. Then it is working. You do not need us. Some families still add neurofeedback with a view to reducing reliance over time, but that is a conversation to have with the prescribing doctor, not a reason to switch.

Medication has not been tried yet and symptoms are severe. Discuss medication seriously with your paediatrician. If it helps, you have solved the immediate problem. Neurofeedback remains available later.

You are firmly against medication in principle. We would still ask you to get a proper assessment first, and to be realistic. Neurofeedback takes months and does not work for everyone. Ruling out the more effective option entirely, before knowing how severe the difficulty is, is a decision worth examining rather than assuming.

Doing both

This is a legitimate option and some families choose it. Neurofeedback does not interact chemically with medication.

One thing to watch: as regulation changes, a dose that suited your child before may start to feel too strong. If that happens it is a conversation with the prescribing doctor. Never adjust or stop medication because training has started.

What we will not tell you

We will not tell you your child can come off medication. That is not our decision, we cannot predict it, and any clinic that promises it is telling you something it has no way of knowing.

We will not give you a success percentage. We do not have one that would survive scrutiny.

And we will not treat this as a moral question. Parents who medicate are not failing their children, and parents who look for alternatives are not being irresponsible. Both are trying to help a child who is struggling.
